Position Overview:

The Support Coordinator is responsible for assisting participants in implementing their NDIS plans, achieving their goals, and coordinating supports and services. This role involves working closely with participants, their families, and other service providers to ensure a holistic and person-centered approach to support.

KEY RESPONSIBILITIES

  • Plan Implementation: Assist participants in understanding and implementing their NDIS plans, ensuring they access the supports and services outlined.

  • Service Coordination: Coordinate and liaise with various service providers, healthcare professionals, and community organizations to ensure seamless service delivery.

  • Goal Setting: Work with participants to identify their goals and aspirations, developing strategies and action plans to achieve them.

  • Monitoring and Review: Regularly monitor the progress of participants' plans, reviewing and adjusting supports as necessary to ensure goals are being met.

  • Advocacy: Advocate on behalf of participants to ensure they receive appropriate services and supports, addressing any issues or barriers they may encounter.

  • Record Keeping: Maintain accurate and up-to-date records of participant interactions, progress, and outcomes in accordance with organizational and NDIS guidelines.

  • Compliance: Ensure all activities are conducted in compliance with NDIS requirements, organizational policies, and relevant legislation.

  • Relationship Building: Develop and maintain strong relationships with participants, their families, and other stakeholders, fostering a supportive and collaborative environment.

QUALIFICATIONS AND EXPERIENCE

  • Education: Relevant qualifications in Social Work, Disability Services, Community Services, or a related field.

  • Experience: Demonstrated experience in a similar role, preferably within the NDIS framework.

  • Knowledge: In-depth understanding of the NDIS and its processes, as well as knowledge of local support services and resources.

  • Skills: Excellent communication, organizational, and problem-solving skills. Ability to work independently and as part of a team.

  • Attributes: Empathetic, patient, and committed to supporting individuals with disabilities. Strong advocacy and interpersonal skills.

OTHER REQUIREMENTS

  • Valid driver’s license and access to a reliable vehicle.

  • National Police Check and Working with Children Check (or willingness to obtain).

  • First Aid and CPR certification (or willingness to obtain).
